Sapphire EHR Virtual Clinical Pharmacy Module

What Is It?
Diamond Pharmacy Services has teamed with Sapphire Health to develop a unique clinical module in an effort to enhance the clinical services currently provided.
Through the Virtual Clinical Pharmacy Module, our clinical pharmacists are able to collaboratively work with onsite providers to provide patient care that optimizes medication therapy and promotes health, wellness, and disease prevention. Additionally, our clinical pharmacists emphasize the core aspects of evidence based, safe, appropriate, and cost-effective recommendations through real time recommendations within the Sapphire Health Electronic Health Record (EHR).
Why Use It?




Example Process

Prescriber opens the Clinical Pharmacist Consultation Request form.
Prescriber selects Initial Consult and the types of consults needed.


Email alert/internal Sapphire message is sent to the pharmacist.
Pharmacist reviews request, completes and submits the relevant follow up form.


Form goes into the Form Response Signoffs queue for provider review.
Prescriber review/signoff


Prescriber follows up with patient or prescriber requests pharmacist to follow up with patient.

Can reduce the workload associated with medication reconciliation and provide appropriate medication administration directions. Focus on minimizing administration frequency, optimizing pill calls per patient, and an added layer of clinical pharmacy education for the nurses allows nurses more time to focus in on other important patient medical modalities.
